What is the Visiting Questionnaire

The visiting questionnaire is a crucial document used in the context of visiting felons in California. It serves as a formal means to gather information regarding the individuals who wish to visit someone currently incarcerated. This questionnaire typically includes personal details about the visitor, the relationship to the inmate, and any relevant background information that may be required by correctional facilities. Understanding the purpose and structure of this form is essential for ensuring compliance with visitation policies.

How to use the Visiting Questionnaire

Using the visiting questionnaire involves several straightforward steps. First, obtain the form, which can often be downloaded from the relevant correctional facility's website. Next, fill out the questionnaire accurately, ensuring that all required fields are completed. It is important to provide truthful information, as inaccuracies may lead to denial of visitation rights. Once completed, submit the form according to the specific instructions provided by the facility, which may include online submission, mailing, or in-person delivery.

Steps to complete the Visiting Questionnaire

Completing the visiting questionnaire requires attention to detail. Follow these steps for successful completion:

  • Download the visiting questionnaire from the appropriate source.
  • Read all instructions carefully before filling out the form.
  • Provide personal information, including your full name, address, and contact details.
  • Indicate your relationship to the inmate clearly.
  • Answer any background questions honestly.
  • Review the completed form for accuracy before submission.
  • Submit the questionnaire as per the facility's guidelines.

Legal use of the Visiting Questionnaire

The legal use of the visiting questionnaire is paramount for ensuring that visitation rights are upheld within the framework of the law. This document must be completed in accordance with state regulations and institutional policies. It serves as a record of the visitor's intent and eligibility to visit a felon. Legal compliance is critical, as failure to adhere to guidelines may result in penalties or denial of access to the inmate.

Key elements of the Visiting Questionnaire

Several key elements are essential in the visiting questionnaire. These include:

  • Visitor Information: Full name, address, and contact information.
  • Relationship to Inmate: Clear identification of how the visitor is related to the incarcerated individual.
  • Background Questions: Inquiries regarding criminal history or other relevant factors that may affect visitation.
  • Signature: A declaration of the truthfulness of the information provided, often requiring a signature.

State-specific rules for the Visiting Questionnaire

Each state may have specific rules governing the visiting questionnaire, particularly in California. It is important to familiarize yourself with these regulations, as they can dictate the information required and the process for submission. For instance, some facilities may have additional requirements for visitors with prior criminal records or may impose restrictions based on the nature of the inmate's charges. Always check the latest guidelines from the relevant correctional institution to ensure compliance.

Waiters waiting for a tip of their preferenceThere was this hotel in Andheri East near the station in Mumbai,India. It was during an afternoon of March 2012, when I was coming out of the coaching class and heading to take my lunch from any restaurant I could find. So I headed out to this one restaurant that serves a regular thaali and south indian dishes. I was drooling over for a masala dosa so enquired of they are serving the dish for lunch. Once confirming I booked a seat in this restuarant and waited for a waiter to take the order. This one guy attends to me and he looked quite distracted with chaos around the restaurant with many coming possibly being lunch time of the day. I had to reiterate my order to him that included the dish, juice and other side items shown in the menu. He took the order, however it took me an rough 40 minutes to get my dish served on my table. This waiter went one dish at a time to serve by placing on my table and heading back to the kitchen to get another. So it takes another 7 minutes and I finally have all I needed. I was so caught up by the chaos and the delay that I took it real slow to complete my lunch. Once done, I immediately signalled for the Bill. As the bill arrives I left a change of 40 INR as a tip and paid the bill, whilst I had seen two other customers putting a couple of 10s and leaving. I thought that atleast the lunch was great and probably the waiter was having a rough time. But to my surprise, the waiter was observing at a distance and rushes to me as I stand up to leave saying “Sahab, sau rupiye toh dho, aaj bahut bheed hai, itne se kya hoga” - Sir , please tip 100 INR as its a crowded time, what will I do with this you have given ? I was like out of courtesy I paid to this much else I would be like others giving 10s and you are asking this much. He starts to act wierd and pitch in the status quo, saying that most “rich” folks should be courteous enough to pay more, as he thinks I look well off , being properly dressed with college bag. The problem he thinks I may belong to the rich families affording to go to college or pay heavy tuition fees, so he can bargain for more. Well I like to dress nicely but I am not from any rich bckground but understanding how things are for such work life as a waiter, I atleast showed some gesture this time. So we go back and forth speaking on status, philiosphy and by them I just stormed out leaving the 40 INR change. I felt it was unethical as if a customer gives you a tip, you graciously accept or decline, either in their presence or absence. I would not appreciate to argue or be adamant to demand more, provided the bill already covers your restaurants services and VAT. I felt strange, however this was just that one incident.

You can get employees at shelters, places of worship, education centers, centers for non-discrimination, job banks, food banks, resource centers, legal aid offices, and many more. I don’t know where you live so I can’t be specific.

I wish that everyone, both liberal and conservative, was able to understand what “unintended consequences” are.10 Fascinating Examples of Unintended Consequences - Toptenz.netLiberals often think that if the INTENT of a policy is NICE, then the OUTCOME of the policy will be NICE.Liberals are NICE. Their NICE eyeballs look out onto the world a see a place where we should all be NICE.But being NICE often has negative consequences.Like letting a child molester or a rapist out of prison is NICE because we want to believe that everyone has some good in them. We want to feel that the person can be rehabilitated. But in reality, 92% of violent criminals will commit the same violent crimes when released from prison. So letting criminals out of jail after a few years is NICE and is also EVIL.Another big delusion is that a politician must have STRONG support to be elected.Sometimes politicians get elected because the opposition got bored and stayed home.America is a nation of centrists, that's why we have close elections. They shrug their shoulders and vote for whoever they sort of like better.Trump can get re-elected even if not one single person in the USA likes him. We have a binary party system, so if the voters dislike Trump less than they dislike Hillary (or whoever), then he'll be re-elected.In a binary political system, a politician is turned either on or off by the voters, like a light switch. Whether the individual voter adores him or hates him but hates Hillary more, the pro-Trump vote counts the same.In the last election, the greatest number of voters voted to stay home and watch Dancing with the Stars on TV. 42% of the eligible voters voted for Dancing With The Stars, 26% voted for Trump and Hillary each. In a pure democracy, the cast of Dancing With The Stars would be president. That's what makes Trump Derangement Syndrome so hilarious. After the election, everyone twirled about in confusion and "realized" that their neighbor Timmy who voted for Trump is a secret FASCIST.It's not that Hillary was an unlikable candidate and ran a horrible, disjointed campaign and got beaten by a blowhard. The fun and exciting version is that "Trump activated his white supremacist base." The same people that elected Obama twice are apparently white supremacists. LOL.I’m not a fan of Michael Moore, but he nailed it:

NOOOOOOO. You are talking to a military romance scammer. I received an email from the US Army that directly answers your question that is pasted below please keep reading.I believe you are the victim of a military Romance Scam whereas the person you are talking to is a foreign national posing as an American Soldier claiming to be stationed overseas on a peacekeeping mission. That's the key to the scam they always claim to be on a peacekeeping mission.Part of their scam is saying that they have no access to their money that their mission is highly dangerous. If your boyfriend girlfriend/future husband/wife is asking you to do the following or has exhibited this behavior, it is a most likely a scam:Moves to private messaging site immediately after meeting you on Facebook or SnapChat or Instagram or some dating or social media site. Often times they delete the site you met them on right after they asked you to move to a more private messaging siteProfesses love to you very quickly & seems to quote poems and song lyrics along with using their own sort of broken language, as they profess their love and devotion quickly. They also showed concern for your health and love for your family.Promises marriage as soon as he/she gets to state for leave that they asked you to pay for. They Requests money (wire transfers) and Amazon, iTune ,Verizon, etc gift cards, for medicine, religious practices, and leaves to come home, internet access, complete job assignments, help sick friend, get him out of trouble, or anything that sounds fishy. The military does provide all the soldier needs including food medical Care and transportation for leave. Trust me, I lived it, you are probably being scammed. I am just trying to show you examples that you are most likely being connned.Below is an email response I received after I sent an inquiry to the US government when I discovered I was scammed. I received this wonderful response back with lots of useful links on how to find and report your scammer. And how to learn more about Romance Scams.Right now you can also copy the picture he gave you and do a google image search and you will hopefully see the pictures of the real person he is impersonating. this doesn't always work and take some digging. if you find the real person you can direct message them and alert them that their image is being used for scamming.Good Luck to you and I'm sorry this may be happening to you. please continue reading the government response I received below it's very informative.   You have contacted an email that is monitored by the U.S. Army Criminal Investigation Command. There are many (many) reasons - so I'll list a few of the ones that I can think of off-hand.Here in the U.S. - we have a multi-party system: Provider-Payer-Patient (unlike other countries that have either a single payer - or universal coverage - or both). Given all the competing interests - at various times - incentives are often mis-aligned around the sharing of actual patient dataThose mis-aligned incentives have not, historically, focused on patient-centered solutions.  That's starting to change - but slowly - and only fairly recently.Small practices are the proverbial "last mile" in healthcare - so many are still paper basedThere are still tens/hundreds of thousands of small practices (1-9 docs) - and a lot of healthcare is still delivered through the small practice demographicThere are many types of specialties - and practice types - and they have different needs around patient data (an optometrist's needs are different from a dentist - which is different from a cardiologist)Both sides of the equation - doctors and patients - are very mobile (we move, change employers - doctors move, change practices) - and there is no "centralized" data store with each persons digitized health information. As we move and age - and unless we have a chronic condition - our health data can become relatively obsolete - fairly quickly (lab results from a year ago are of limited use today)Most of us (in terms of the population as a whole) are only infrequent users of the healthcare system more broadly (cold, flu, stomach, UTI etc....). In other words, we're pretty healthy, so issues around healthcare (and it's use) is a lower priorityThere is a signNow loss of productivity when a practice moves from paper to electronic health records (thus the government "stimulus" funding - which is working - but still a long way to go)The penalties for PHI data bsignNow under HIPAA are signNow - so there has been a reluctance/fear to rely on electronic data.  This is also why the vast majority of data bsignNowes are paper-based (typically USPS)This is why solutions like Google Health - and Revolution Health before them - failed - and closed completely (as in please remove your data - the service will no longer be available)All of which are contributing factors to why the U.S. Healthcare System looks like this:===============Chart Source: Mary Meeker - USA, Inc. No country will let a visitor in if they believe that person intends to be homeless. The ability to look after yourself and not be a burden on the state is one of the reasons visitors are screened in the first place. No, you can't leave it blank when you are trying to enter a country as a visitor. If you are returning home however, you're not a visitor and in my experience no country seeks address information for citizens returning home.

I’m surprised nobody has commented on lying at MEPS. You have two choices. You can either do what your recruiter told you to do, and LIE to everyone, or you can tell the truth. The decision you make decides the outcome of your entire Enlistment experience. Think of this like one of those choose your own adventure stories. I’m going to tell you how both go, since I had the honor of going through MEPS twice.Option 1. You tell the truthSo I first enlisted to join the Marines, and my recruiter, shady as he was, told me to be honest while I was in MEPS. So when I got to the questionnaire where you have to fill out your medical history, I was honest…I wrote down that I had headaches sometimes, vertigo, had been depressed for a while, thought about suicide (because what teenager hadn’t at one point in High School), etc. I was pretty damn honest. Well, The doctors weren’t having it I guess because I got disqualified based on past history, and couldn’t join.Option 2. You lie.So my second go around, I decided to join the Army. This time, my recruiter tells me to lie…”I don’t care if you’ve got AIDS, Cancer, etc…just say you’re normal so you can get in”. So that’s what I do. every box on every form that had something negative (history of heart problems in family history? Nope. History of Depression? Nope). No NO NO NO NO NO NO. Read the big ass sign on the wall saying “Failure to answer truthfully can result in jail/fine/etc…Signed my name and was done. I shipped off to basic training a few months later. You need to decide for you how your MEPS experience goes, try to be honest, because certain things (like your chronic Asthma) they’ll find out sooner or later…and if they find out you lied once you’re in Basic or After, the punishment for lying will be a lot stiffer.Good luck…I wont rehash what others wrote…MEPS is an ALL Day affair, like 5AM - 5PM. sometimes it can take multiple visits…it’s cold, you get shit food, and it makes the DMV look like paradise.
