Oh, we read them. We base the start of your plan of care on them. As the nurse doing that, I go over everything. The doctor I work for uses it to be sure he covered everything. It's very common to forget something when you have the doctor in front of you. This is my profession and even I do it. We expect you to forget something.Then it gets scanned into your chart, there, forever. I refer back to those forms if, for example, your labs turn up something life threatening and I can't signNow you. Who was that you listed as an emergency contact? Hope it's legible. Heck, I hope it's filled out! ( If it was entered before my time, it might not be. Now, you can't see the doctor without it filled out.)It's so important my practice asks you to re do them every year. Patients hate it, complain about it, loud! But if I had a dollar for every time I couldn't signNow someone in this day and age of fluid phone numbers, why, I'd have several more dogs and we'd all be living somewhere warmer!And…oh, you have another doctor? We didn't know that. And they prescribed what? Did what tests? We don't know if you don't tell us 99.9% of the time. You would be amazed how many patients don't bother to tell their primary care physician such important things like…they went to the ER, had an MRI, see a cardiologist, and..etc and so on. We don't automatically know. We should, but that's another story.Feel that paperwork is beneath you? Are you too busy to fill it out? I see that every day too. You know what that tells us? That you don't value this very much. That you are so much more likely to be non compliant, not take meds, no show for appointments, maybe fib a little….a lot… your lifestyle choices….how, if you take your meds. I mean, come on, you can't even follow directions to fill out paperwork! How do you expect us to take you seriously, when from the very start, you don't offer us the same courtesy.If there are any doctors out there, not reading these things, shame on you. But in 30+ years, I have not seen it. As for the doctor asking you about it, well, mine works very hard to get it right. And even the most earnest patients forget something.

I was actually roommates with someone in College whose parents decided to have him “off-grid”. Let me just say this: Stop even entertaining the idea of having a baby off-grid. It really makes your child’s life unnecessarily hard and your kid will forever resent you for putting that pain on them.I’ll get into the details in a moment, but first let me address the question proposed above. Yes, technically speaking it is not a crime to have your baby off-the-grid in the U.S.A. However, a lot of details surrounding the event would be illegal. First of all, any licensed midwife or doctor is required by law to file a birth certificate or they actually risk losing their license and getting a misdemeanor. If you forced them or threatened them to not file the birth certificate that would make you a likely accomplice and would not go over well with the authorities.But let’s ignore that for a moment and just assume you know how to birth a child on your own and can do it in your basement without any professional medical physician there to oversee you (which would be the only way you could pull this off). In this case you wouldn’t get thrown in jail for failing to get a birth certificate and no crime would have been committed. However you just set up a very difficult life for your child.These are some of the things I was told from by my roommate who didn’t have a social security number until he was 20 years old.No, he could not get a (legal) jobQuite literally he didn’t qualify to get even a job at McDonalds. If you remember the last job you got no matter how prestigious or demeaning it was, you had to fill out a bunch of paperwork. Most of these forms require you to have a SSN (social security number) to properly fill them out. However the important one is the form labeled I-9. This form is required to be submitted by every employer after hiring a new employee. This form serves only one purpose, to determine that you are eligible to work in the United States. Your child (and my roommate) would not be able to complete this form which every employer must get filled out before starting employment with a new employee. Your child will not be able to get a job because of this.Yes, he can evade paying taxes.Okay, so this sounds like a perk I guess. But my roommate did not have to pay taxes. The government basically didn’t know he existed, so they never knew he was not paying. But then again he didn’t have a job. So would you rather have a job and pay some taxes or not ever be able to work except under the table for below minimum wage? Given that choice, taxes sound pretty awesome! Keep in mind that this also means your child is not eligible for any tax benefits or credits such as those that students get while going to college.No, You as the parent can not claim him as a dependent on your taxesYou’re already dealing with a child, wouldn’t it be great to get that child tax credit? Every year you'll basically be paying out of pocket for deciding not to get them a SSN.Yes, he can attend public school through 12th gradeHe would be able to attend school through high school without a social security number.No, he can not attend collegeWhile high school and lower education is okay, your child will never be able to attend collegeYes, he can go to the doctorThe doctor will still see your child and provide him his shots. This also will disqualify him from renting most homes or apartments he is looking at.Basically your child will be treated as an illegal immigrant. Why put them through this when they are entitled to the benefits that the United States provides its citizens? There are people in other countries dying to get what your child is entitled to and you are (considering) denying your child those abilities? It just doesn’t make sense.Get them a SSN and if your child decides at 20 that he wants to live off-the-grid than he can burn his Social Security Card and go in the woods and hide from the government. But don’t be selfish enough to make that decision for them.My roommate resents his parents for not giving him a SSN. While all his friends in high school were driving, he couldn’t. While his friends go to Cancun for spring break, he had golden handcuffs in the U.S. and can’t leave. And worst of all he said was that while all his friends were earning money from jobs in high school, he couldn’t get a job.The job part was the hardest for him. He couldn’t leave the house or move out when he turned 18. He was stuck at home.Him working on getting a social security number was difficult and took him two years. He started when he turned 18 to get one once he realized he couldn’t go to college, he couldn’t get a job, he couldn’t rent an apartment, and thus will never really be able to be independent from his parents. It took two years and then at 20 he was able to get one and start working and going to school.He forever resented his parents. Don’t be those parents…

Am I the only combat medic to answer this so far?Yes you can. Medics are taught how to but not instructed to. There is a terrible and fine line out there in “the suck” that medics, and medics alone, are asked to walk.You don’t end a person’s life. Full stop. In the rare case that a soldier is mortally wounded (no way to maintain an airway or control bleeding and no higher medical assets within a reasonable time)… then a medic could administer an extra ampule of or two. Even though the doctors and instructors teach the medics this, in the end it’s on that one person’s shoulders. And conscience.Is it better to leave your friend/co-worker screaming in agony until they are too weak to yell? Then watch them convulse every few minutes for a couple of hours. Then finally they stop responding to your voice or even painful stimulus.Brain death is setting in. It takes a few minutes or a few days.Every minute you have a seriously wounded soldier in your unit you have medics that are out of the fight. You also have a much more complicated command situation. Nobody (NOBODY) makes this decision lightly. They also never talk about it.In the movies there is always an EVAC helicopter with escort available and ready to risk anything to get to the wounded. In combat it’s not always possible. “Birds” get grounded for many reasons and MEDEVAC Strykers are delayed by the need for escort vehicles/crews and IED laden roads. In almost all cases, the wounded will live to see the operating room. In some form.Combat wounded are intense. Gunfire is still raging in many cases. People are yelling, confusion is everywhere. The medic will be well trained but under a lot of stress. They know that they have to address breathing and bleeding in 2–3 minutes. They also need to avoid causing further injury and find any hidden wounds. While doing this they have to coordinate any available soldiers with combat lifesaver training to assist them with this or other injured. Finally, they also have to constantly keep the command apprised of the situation.Who has X injury?Can they return to the fight?Do they need to be evacuated from battle or can we take them with us?If they need to go NOW, how long do they realistically have?Can we ground evac through the combat or do we need a bird?While answering all of that the medic has assessed the wounded. Tried to control the bleeding and established a secure airway. Then they need to find a vein for an IV (super hard on a patient with blood loss or missing limbs). While doing this they also need to fill out the ‘9 Line’ medical evacuation form for the radio. Once this is done the medic will check the field dressings, the IV, the breathing. Record the wounds and vitals. Mark when/if was given (how much, when, where administered) and done so that the surgeon can see it and blood doesn’t wash it away. Often in black sharpie on the forehead if patient is unconscious- as awful as that sounds it works well.So, don’t talk about the morality of this until you walk a mile (or 26) in a medic’s boots. Don’t talk about what happens until you live and work with a small team of men and women in a combat zone for over a year at a time. Infantry units are closer than most marriages/families. Your platoon SGT is dad and doc is mom. It’s a horrific moment to see one of your guys literally torn in half and dying. It’s much worse to know that due to a sandstorm there aren’t any flights that day. It’s hell on earth when you realize nobody is coming by road because of the IED you just hit. It’s unimaginable when you realize you only have 2 ampules left and 3 critically wounded friends.I didn’t have to make the hardest choice. I wouldn’t tell you if I did. I sure as &$*# wouldn’t take any judgement from you in any case.Great question. I hope someone who actually held this responsibility in combat can clear it up a little.

I have to go anonymous on this because my wife is not yet comfortable sharing our experience during the "childless years". I don't understand her reasons at all but I have to respect her wishes. (I guess there is stigma associated with infertility in Asian countries).Anyway, we married when I was fresh out of grad school at 23. So we chose to have a few years to financially and academically(wife finished grad school) settle down. That was 5 years. At 28 we figured its time. And it didn't happen. For a long time. My wife was on Deprovera as it was convenient but getting off that and becoming fertile again is said to take upto 18 months. 2 yrs passed by and then it began. The fertility tests and doctor visits and attempts. In the next 3 yrs, we went through 11 IUIs, 4 IVFs with ICSI. After the 3rd IVF at a well advertised fertility clinic in the Bay Area (Daly City), the doctors told with quite a bit of certainty that we could never conceive.During those years, we had more than enough money (double income no kids), a house we built from a fixer-upper to a perfect family home and a big huggable rottweiler and all we needed were some kids. We were not US Citizens, which made any kind of adoption scenario (US or Overseas) about 3 years away. So we were just stuck being childless until either we got pregnant or the adoption process could start.How did it feel? Mostly, EMPTY. We traveled, we dined, we took hikes, we rode motorcycles and convertible cars for long distances over scenic roads. They all were very romantic, fun and enjoyable but simply distractions. His holiness, The Dalai Lama's books helped and we were committed to simply spreading as much positivity as we could. Money, Food and Blood donations. Always being consciously compassionate and helpful to other without asking/expecting for anything in return.In essence all those "childless years" helped us become compassionate, helpful and generous people.Then it happened.. mostly due to my wife's perseverance and relentless faith. We took the notes and recommendation from the doctor who did our 3rd and 4th IVF in San Francisco and took it to a good doctor in India (we had ran out of money for $20K IVFs in US) for one final attempt.It worked!! Beyond our wildest imaginations and as a true definition of a MIRACLE, it happened. I don't believe in a conventional God or religion but I believe in Karma and balance. I think all those good Karma points paid off.Then when we asked for nothing more in life... she got pregnant again.... conventionally... It was a Saturday and we went to the clinic in disbelief. They confirmed and our heads were spinning...Now we had 2 kids and a full happy house...but wait.... AGAIN!?! All those stories about failed contraceptive methods are true. Happily true.As of today we have 2 boys and a girl who drive us crazy and make every muscle in our bodies physically hurt. But, we are eternally grateful to the universe and we are living in a miraculous life. A life we had never imagined in our wildest dreams. And we became better humans through that difficult experience of "childless years". I think it has also helped us being more patient and thankful parents.[Even if we didn't get pregnant we'd still have adopted kids, as a certainty].I cannot imagine spending an entire life without being a father. (I respect people's choices and don't judge them, but I personally don't understand a conscious choice of staying childless against nature.)Ps: 2 things I want to get out there. One, any kind of same species parenthood is awesome and its a natural responsibility. Go for it, be it biological, adoptive, foster or god-parenting. Two, cats and dogs and other pets are not ultimately a substitute for human children, no matter how much mutual love and care there is.Edit for “Child free” readers: As a few commenters have pointed out how my answer hurts them as apparently I am judging their choice. This question is “how is it remain a childless couple” and I have answered about how I felt when we were Childless. I stand by my statement that if a couple is physically, financially and emotionally capable to be a parent, biological is not the only way to get that joy. There are so many kids in the world who will love to be part of your family and call you Mom/Dad.My answer has nothing to do with “Child free” people as I DON’T UNDERSTAND. It is a natural and ethical rights of students of respective states.C) If the private medical college seats are meant for the respective states only, then there should be one all India Rank for 15% of all India quota in Government Medical colleges across the country and one state rank for private medical college seats, other than the Management Quota seats, of respective states.D ) If the state rank is given for the seats of private colleges of respective states, then who will coordinate and allot the seats. There should be single window system like CET or NEET.E) Because, in Karnataka COMEDK has invited application separately from the NEET qualified candidates at all India level to register online by paying somewhere around Rs.600.00 -1000.00 for the seats in the colleges comes under its fold. It is once again one’s own responsibility after qualified in NEET, notwithstanding, his position of NEET ranking has to register afresh. The very purpose of NEET was to avoid multiple competition and spending money for multiple registration and wandering of parents and children for counseling from college to college.F) Apart from COMEDK there are many other medical colleges comes under Private universities and linguistic and religious minorities associations .It seems, Many of them have been told ( not officially known yet) to register individually with all those institutes separately by paying the registration fee somewhere around Rs.1000.00 to 1500.00 once again and apply with NEET rankings. Then how the purpose of NEET shall safeguard the interest of the meritorious candidates according to their order of NEET rankings, if the private colleges are enrolling for the counseling in their colleges separately. In such case, one may easily guess that the order of NEET ranking will be nothing to do with one to get seat, why because the higher ranking candidates may miss the opportunity for the lower rank because he may not be capable of applying for all these colleges separately.G) If the seats of all private medical colleges across the country are open to all India quota and each student need to register separately with each college, as the situation prevails now, there are not less than 250 private medical colleges(according to MCI total 400 + medical colleges – Govt Medical Colleges) in the country and one has to pay Registration fee more or less around Rs.2,50,000.00, at an average of Rs.1000.00 per college, in addition to the traveling cost of attending the counseling at different colleges at different places.H) Is it possible to the candidates to attend the counseling personally across the width and breadth of the nation with in the scheduled time frame and what to do in case of overlap dates of counseling of different colleges at different places.I) The fee in the private medical colleges is very expensive for the poor, lower middle class and middle class people, kindly consider the same to bring in to the limits of affordability as the professionals are going to safe guard the health of the citizens of this nation which is most important and precious.Thanking you,Obediently yours,

Trust me, I lived it, you are probably being scammed.
