The degree to which you do so is up to personal preference but you should AT LEAST have an adventure hook, a starting point, an ending point/boss battle, and a few major events in between. The players might skip parts, that’s fine, but you need to have some kind of logical progression. You can get pretty detailed about it if you want, just keep in mind the players might never see it depending on what they do.Here’s an example.Starting point/adventure hook: The mayor of Darebrook has posted a bounty on a nearby orc clan which has been causing problems. For various personal reasons, a group of adventurers has come together to bring them in.Main Event: the group has to find out where the orc clan is hiding. They can do this by asking around in the town, finding a Ranger Guide NPC you’ve planned out, buying a map, going to their last raid site and tracking them, etc.Main event: say they go to the last raid site. There will be an encounter there with a goblin vanguard allied with the orcish clan. If captured, a goblin can lead you to the orcs’ last known hideout, or the orcs can be tracked once the goblins are defeated.Main event: so they’ve made it to the orcs’ former hideout, but it’s deserted. It’s a medium cave system (DRAW A BASIC MAP) which, if entered, is found to be full of a number of traps, dead bodies, and detritus left behind when the orcs ran for it. Additionally, the cave is full of whichever creature you choose that drove the orcs out. Maybe they were ran out by a rival tribe of hobgoblins and bugbears, or a group of trolls or ogres, angry hill giants, a giant remorhaz, a dragon, etc. By defeating/capturing them or by making investigation checks and tracking them, the party can find out in which direction the orcs ran - the main stronghold of the clan. They also might run into a raiding party from the main orc stronghold which can lead the party there, if they chose not to go into the deserted hideout.Main event: after traveling through the wilderness, they’ve come to a ruined castle (MAP) which was commandeered by the main bulk of this orc clan. The group you need for the bounty is likely somewhere inside. Waiting a while might let the players see raiding parties entering and leaving, but the bountied orcs are not among them.Main event: after making their way through the castle, encountering one or two bountied orcs and a horde of others, they run afoul of the warlord who wants to kill them for honor and put their heads on pikes (who happens to have a separate and far greater bounty on his head from the lord of the region). Boss battle. The other bountied orcs as well as a large number of the clan joins in.Ending: after crushing the seat of power of this orc clan, they return and collect the bounty for the original group of orcs. If they killed the warlord, the lord of the territory rewards them handsomely (perhaps even with noble titles, deeds to land, magic items, and other great rewards).There’s a campaign which would take my group multiple sessions to finish, which I planned in under half an hour. I would bring this outline to my session, written down on notebook paper, and refer to it whenever I’m getting lost. Detailing basic plot points like this gives your players choices without forcing you to improv everything on the spot.DON’T start a long running campaign without at least a general idea of where the story will go, unless you’re not taking it seriously (because it will go weird places after you’ve run out of ideas for the fifth time).Good luck. How would others react to this person? What are their goals? What makes them a villain? How do they justify their actions? See what comes to you, and write it. Don't try to be clever. Try to tell a good story. If this is the twist you want, keep it at the heart of your story and your character's motivation, let it guide you as you write. But it has to be your story. No one can tell you how to do it. All that said, you may want to read some articles on Unreliable Narrators, and some stories with unreliable narrators (Tell Tale Heart comes to mind,) as that seems to be the sort of thing you're going for. Just remember, writing a story isn't about showing off how clever you can be or creating the most amazing twist. It's about telling a good story, a story that means a lot to you and that you can be excited about. Write for yourself, not for acclaim, and you'll do just fine.
